For school enrollment, a parent or guardian shall provide one of the following:

1. Current, up-to-date immunization records; OR

2. A completed and signed exemption form

Students with medical contraindications, religious objections, or personal objections to immunization may complete and submit the Immunization Exemption Form linked below.

Note: According to Oklahoma state law, a student’s childhood exemptions expire at the end of their sixth-grade year. All students with an exemption must reapply before enrolling in seventh grade.
